Intrauterine ınsemination is a natural fertility treatment that may increase chance of pregnancy in couples who fail or have difficulty in conception. Intrauterine ınsemination is an office procedure in which prepared sperm cells are placed directly into a womans cervix or uterus to produce pregnancy,with or without ovarian stimulation to produce multiple oocytes. IUI can also be preferred for single woman,who use donor sperm,couples with male-factor infertility,such as low sperm count.

How is intrauterine insemination performed?

What happens after iui day by day?

Monitoring for Intrauterine ınsemination is started on day 2 or 3 of the menstrula cycle.Once collected, the semen sample is then “washed” in the laboratory, to concentrate the sperm and remove the seminal fluid (seminal fluid can cause severe cramping in the woman). This process can take up to two hours to complete.

IUI is performed near the time that the female partner is ovulating. The IUI procedure is relatively simple and only takes a few minutes. The woman lies on an examining table and the clinician inserts a speculum into her vagina to see her cervix. A catheter (narrow tube) is inserted through the cervix into the uterus and the washed semen sample is slowly injected. Usually this procedure is quite painless, but some women have mild cramps.

Timing
Timing the IUI is very important – it must be done during the “fertile period” when the egg is in the fallopian tube. Pinpointing the time of ovulation accurately using either vaginal ultrasound or ovulation test kits is crucial. A good clinic should provide this as a 7-day week service, since there is a 1 in 7 chance that ovulation will occur on a Sunday – eggs don’t take a holiday! It is important to superovulate the wife at the same time (with clomid or HMG injections) , so that she produces more than one egg. Superovulation increases her fertility potential as well, thus increasing the chances of conception by improving the chances of the eggs and sperm meeting.

The IUI is done either when ovulation is imminent or just after. The husband masturbates into a clean jar – preferably in the laboratory or clinic itself, and after at least three days of sexual abstinence to get optimal sperm counts. Some men may have considerable difficulty producing a semen sample at the appropriate time, because of the tremendous stress they are under, and the ” pressure to perform”. For these men, using a previously stored frozen sample can be helpful. Viagra (sildenafil citrate) can also be used to help them to get an erection, as can using a vibrator.

Success rate Intrauterine ınsemination?

The success rate for the procedure depends on several factors, which include age and underlying fertility challenges. Let's explore IUI success rates within these two segments. 

Intended mother's age
As a woman ages, the conception rates drop due to egg quality. Because of this, IUI isn’t usually recommended for intended mothers over 40. 

For women in their early 30s or younger, the success rate is around 20-25%
For women aged 30 to 35, the success rate of Intrauterine Insemination is typically 15-20% 
Women aged 35-40 have a 10% success rate of becoming pregnant after IUI. By their early 40s, the success rates drop to approximately 5%
Underlying fertility issues 
Women who have irregular menstrual cycles or don't ovulate need medication to conceive. Through a diagnosis, the doctor may identify issues that can affect your success rate.

If you are experiencing unexplained fertility with healthy eggs and two fallopian tubes, IUI success rates are around 7% to 10% per cycle. If you use IUI treatment in combination with fertility medications your success rate increases by 15-25%.
A single open fertility tube means one of the fallopian tubes is blocked. Pregnancy can occur but the success rate depends on the location of the blockage. If the blockage is near the ovary, the IUI success rate is 11.7%. If it's close to the uterus, the chances of success are around 38.1%.
If the intended father is experiencing male factor infertility, IUI success rates are around 16.9%.

Is it a painful procedure?

Intrauterine(artificial insemination-ıuı) ınsemination has no diffrence from a normal gynecological exam.It does not requre anesthesia and it isi pain-free procudere.The patient may resume a routine daily life 30 minutes of resting.

How many times can IUI be attemted?

Three to six IUI cycles may be offered before considering alternate therapy in Turkey. However, patients with advanced maternal age, severe male factor infertility, tubal pathology, or severe endometriosis will benefit from a direct referral to IVF/ICSI.

When does pregnancy occur following Intrauterine ınsemination ?

If you do not get a period after 17 days, a pregnancy test will be done. If the treatment has been unsuccessful, you will start a period about 9-14 days after insemination. About 2 weeks after a positive pregnancy test, we will arrange for you to have a scan to confirm the pregnancy and then you will be referred to the antenatal clinic.
If you have a period you will need to telephone the hospital to arrange a review appointment and discuss further treatment cycles.

Intrauterine ınsemination cost in Turkey ?
